Why Blood Inventory Management Remains Fragmented – And How Digital Networks Can Help

Blood transfusion services have saved millions of lives over the past century. Yet, despite remarkable advances in transfusion medicine, one aspect of blood management remains surprisingly fragmented: knowing where blood products are available when they are urgently needed.


Over the past year, I have had the opportunity to discuss blood inventory management with clinicians, blood services, hospital transfusion teams, and digital health leaders across UK, Australia and Southeast Asia. One observation has become increasingly clear: while every country shares the same goal of ensuring safe and timely access to blood products, the way blood inventories are managed varies considerably.


There is no universal model. In the United Kingdom, blood collection and distribution are highly centralised through NHS Blood and Transplant. In Australia, blood collection is coordinated nationally through Lifeblood, while hospitals manage their own inventories using local laboratory information systems. In countries such as Thailand, Malaysia, and Indonesia, hospitals, regional blood centres, and Red Cross organisations each have different responsibilities depending on the structure of their healthcare system.These differences are not weaknesses; they reflect decades of healthcare policy, geography, funding, and local operational requirements.


However, they also create challenges.When hospitals need additional blood products, identifying where stock is available often depends on existing communication channels, local knowledge, or established referral pathways. While these processes work well during routine operations, they may become more difficult during periods of increased demand, natural disasters, or mass casualty incidents. The challenge is not simply having enough blood. It is having timely visibility of where blood is available.

Most laboratory information systems are designed to manage inventory within individual hospitals. They perform this function extremely well but are not intended to provide regional or cross-hospital visibility. Even where technical integration is possible, hospitals often use different laboratory systems, update inventory differently, and operate under different governance arrangements.


Through discussions with hospitals and blood services, several themes consistently emerge.First, any system displaying blood inventory must be restricted to authorised users. Blood stock information is operationally sensitive and should not be publicly accessible.Second, interoperability is more than a technical challenge. Integrating multiple laboratory information systems requires common data standards, reliable interfaces, and agreement on how inventory is updated and shared.Third, governance is fundamental. Hospitals need confidence that inventory information is accurate, that each organisation retains control over its own data, and that any shared information supports existing clinical and operational workflows rather than creating additional complexity.

These considerations are not barriers to innovation; they are essential requirements for successful implementation.Rather than replacing existing blood bank or laboratory systems, there is an opportunity to complement them.Hospitals with blood banks or transfusion services could independently use digital tools to improve local inventory visibility. Where multiple hospitals choose to participate, they could also form a collaborative network, allowing authorised users to view blood availability across participating sites while each hospital continues to manage and control its own inventory. Such an approach could support regional collaboration, improve situational awareness during shortages, and assist retrieval teams when rapid decisions are required.Importantly, the solution is unlikely to be identical in every country. Healthcare systems differ, and digital platforms must adapt to local governance, infrastructure, and clinical practice rather than expecting health services to change established workflows.

Rather than attempting to replace existing blood management systems, our aim is to complement them by improving inventory visibility, supporting collaboration between participating hospitals, and strengthening preparedness during both routine operations and emergencies.


The future of blood inventory management is unlikely to depend on a single national system or a single technology. Instead, it will rely on interoperable, secure, and clinician-centred digital tools that connect the right information with the right people at the right time.Because in transfusion medicine, knowing where blood is available can be just as important as having it.
