ICU Audit – Tracheostomy is a structured procedure audit tool, available as a Google Form and printable PDF, designed to support governance, safety, and quality monitoring of tracheostomy practice in the Intensive Care Unit.

The audit captures key patient, procedural, and post-procedure variables, including:

  • Patient demographics and location of procedure

  • Pre-procedure laboratory results and medication status (antiplatelets and anticoagulants)

  • Airway assessment and procedural details

  • Use of adjuncts such as video laryngoscopy and bronchoscopy

  • Post-procedure checks, imaging, and arterial blood gas results

  • Documentation of complications

The tool enables consistent documentation and review of tracheostomy practice, supports identification of risks and variation, and assists ICUs in meeting clinical governance, safety, and audit requirements.
