ICU Audit – Patient-Reported Outcome Measures (PROMs) is a structured follow-up tool, available as a Google Form and printable PDF, designed to capture patients’ health outcomes after discharge from the Intensive Care Unit.
The questionnaire focuses on patient-reported outcomes across key domains, including:
Overall physical health following ICU discharge
Ongoing physical symptoms or limitations
Mental and emotional well-being
Recovery support and unmet care needs
Overall perception of recovery and outcomes
PROMs provide valuable insight into how patients feel and function after critical illness and complement clinical outcome data by incorporating the patient perspective into service evaluation and quality improvement.
Responses are collected confidentially and analysed in aggregate to protect patient identity. Data are used solely to support audit, service evaluation, and improvement of post-ICU care and recovery pathways.
The tool can be completed electronically via a secure Google Form or in paper format using the PDF version, making it suitable for routine follow-up, quality improvement programs, and research projects.
