ICU Audit – Central Venous Catheter (CVC) Line is a structured procedure audit tool, available as a Google Form and printable PDF, designed to support safe insertion practices, complication monitoring, and clinical governance in the Intensive Care Unit.

The audit captures key elements of CVC insertion and post-procedure care, including:

  • Patient demographics and location of procedure

  • Pre-procedure laboratory results and medication status (antiplatelets and anticoagulants)

  • Insertion site, side, and number of attempts

  • Use of ultrasound guidance and confirmation of guidewire removal

  • Catheter length, securing method, and position confirmation (chest X-ray, venous blood gas)

  • Documentation of complications such as bleeding and pneumothorax

The tool enables consistent documentation and review of CVC practices, supports identification of variation and risk, and assists ICUs in meeting safety, infection prevention, and governance requirements.

Data can be collected electronically via Google Forms for streamlined reporting or via the PDF version for paper-based audits, making it suitable for routine audit cycles, education, and quality improvement initiatives.
