ICU Audit – Patient-Reported Experience Measures (PREMs) is a structured feedback tool, available as a Google Form and printable PDF, designed to capture patients’ experiences of care during their stay in the Intensive Care Unit.
The questionnaire focuses on key aspects of patient-centred care, including:
Respect, dignity, and privacy
Clarity and quality of communication
Involvement in decision-making
Emotional support provided by ICU staff
Information given about treatment and recovery
Overall satisfaction with ICU care
Responses are collected anonymously and analysed in aggregate to protect patient confidentiality. Feedback is used solely to support service evaluation, quality improvement, and enhancement of patient-centred care within the ICU.
The tool can be completed electronically via a secure Google Form or on paper using the PDF version, making it suitable for routine patient experience audits, accreditation activities, and research projects.
