Step 1 

To access the Google Forms, click on the buttons below and you will be directed to the form.

Step 2

You will be asked to make a copy.

Once you are on the form page, look for the three horizontal lines (hamburger icon) located on the top right corner of the page, next to the "Send" button. Click on it, and a pop-up will appear asking you to make a copy of the form.

Once you have made a copy, it will be saved on your Google Drive for future reference.

Step 3

Next, go to your Google Forms and open the form that you saved. Right-click on the form to rename it.

Step 4

To find the green spreadsheet icon, click on the Response tab of the form, located in the right-hand corner of the form. It is recommended to create a new spreadsheet, as all responses will be saved there.
