Procedure Log
 Maintain logs of procedures ranging from central lines to ventilation performed in Intensive Care Unit or Emergency Room


Instructions to use the forms

Step 1

After you click the image, you will be prompted to make a copy of the form. A copy of the form is saved on your Google Drive.

Step 2

Go to your Google Forms. Open the form that you saved. Right click on the form to rename the form.

Step 3 

Open the form. Feel free to customise the form.

Click preview and start filling out the form. The form can be shared with others and responses can be collected. 

Step 4

Click on the Response tab of the form to find a green spreadsheet icon on the right-hand corner of the form.

Step 5

It is recommended to create a new spreadsheet

Step 6 

All responses will be saved on the spreadsheet
